Clearance Requirements for Entrance into Clinical
Prior to beginning 300 level courses, all students are required to complete
clearances. These clearances are mandated by the clinical agencies. There
is no opportunity to waive or delay the attainment of these clearances. Students
who are non-compliant are not permitted to progress into 300 level nursing
courses.
Deadlines for clearances are July 1 for the fall semester and December 1
for the spring semester.
In order to complete clearances, all students must enroll in the American
Data Bank immunization tracking service and pay a yearly fee to maintain
their records at www.holyfamilysafety.com.
All documents are submitted to American Data Bank. No records are submitted
to the program chair or faculty in the nursing department. Requirements include
a clear FBI and state background check, a clear child abuse background check,
a current American Heart Association CPR BCLS for Healthcare providers certification,
personal health insurance, physical exam, titers showing immunity to communicable
diseases (please see student handbook for clarification), two step PPD, yearly
flu vaccine, Hepatitis B (3 shot) series and titer, as well as other clearances
outlined in the BSN Student handbook.
Students can obtain forms to take to their physician on the American Data
Bank site created for Holy Family, http://www.holyfamilysafety.com/ .
Students should first read the requirements in the handbook, then go to the
