
All entering the students residence are required to submit to the
vice president for student services a detailed confidential medical
history and physical form
provided
by the University. Registration will not be completed for students who
do not have a completed form on file in the health services office.

What is a Proxy Form?
The Office of Residence Life has set up a process
whereby students who are off-campus OR who are unable to attend room
selection can select a room by proxy. A proxy form allows you to
designate someone to select housing for you in your absence. Advance
registration MUST be completed and the student's account MUST be
current (paid and up-to-date) in order to allow the designated proxy
permission to pick up a Housing Contract and participate in any housing
selection session. Students are also welcome to contact members of
the Residence Life Committee who could choose housing for them at
the time their lottery number is called.
